HGV/LGV/PCV

 Each medical takes about 15-20 minutes and involves an eye test, completion of a medical questionnaire and a short examination (blood pressure, height and weight).

 

What to bring:

  • D4 form available from the DVLA (this can be downloaded using the link Order DVLA form-GOV.UK). Please fill in your name and date of birth in the box at the bottom of each page. Also, fill in your GP's details on the front page and details of any hospital consultants you are currently under on page 7

 

  • Photo ID (passport or driving licence)

 

 

  • If you wear glasses for distance vision (not reading)  bring them with you and a copy of your most recent optician's report. Contact lens users should wear their glasses for the medical

 

  • A list of any current medication with doses

 

  • If you are diabetic on insulin or injectable treatment bring a record of three continuous months of blood glucose readings on a memory meter

 

  • Payment is by cash or cheque (made payable to "Catherine Colvin") for £55. Receipt supplied
